Fundamental Principle: “First, not harm” is the core of healthcare services, yet avoidable patient harm is a global issue with significant human, moral, ethical, and financial implications.
Definition: Patient safety is the absence of preventable harm and the reduction of unnecessary harm risks to an acceptable minimum.
Organized Activities: Creating a framework of activities, cultures, processes, and technologies that consistently lower risks and reduce harm in healthcare settings.

Common Sources of Patient Harm
Data from WHO: Medication Errors: Affect 1 in 30 patients, with a quarter of cases being severe or life-threatening.
Surgical Errors: 10% of preventable harm occurs in surgical settings, particularly pre- and post-surgery.

Healthcare-Associated Infections: Lead to extended hospital stays, disability, antimicrobial resistance, financial burden, and avoidable deaths.
Sepsis: 23.6% of sepsis cases in hospitals are healthcare-associated, with a 24.4% mortality rate.
Diagnostic Errors: Occur in 5-20% of physician encounters, with 0.7% of adult admissions involving harmful errors.

Patient Falls The most frequent adverse event in hospitals occurs at a rate of 3-5 per 1,000 bed-days, often leading to injury.

Venous Thromboembolism: A preventable cause of patient harm, contributing to a third of hospitalization complications.
Pressure Ulcers: Affect more than 1 in 10 hospitalized adults, with significant impacts on mental and physical health.

Unsafe Transfusions: Lead to serious adverse reactions and transfusion-transmissible infections, with 12.2 serious reactions per 100,000 distributed blood components.

Patient Misidentification: Causes serious adverse effects like wrong-site surgery, with 12.3% of sentinel events linked to misidentification.

Unsafe Injection Practices: Risk of infectious diseases like hepatitis and HIV due to unsafe injections, with significant global health implications.
